REQUIRED FOR ALL STUDENTS
NEW STUDENTS
Bring the specified documentation in categories I, II, and III of the
residency verification documentation to the school office or
Zion District 6 Administrative Office.
RETURNING STUDENTS
Submit residency and/or address change documentation through our online form below.
2023-2024 Residency Documents & Address Change Form
Notary services are available at the District Office (2800 29th St, Zion, IL 60099) if required.
The parent/guardian along with the owner or lessee must be present for notarization.
Residency Verification Documentation
In addition, NEW students must also bring a birth certificate and Illinois Transfer Form (if coming from another Illinois school).
A hospital certificate is not sufficient...We will make a copy and return your original.
