Residency Affidavit

If the parent/guardian is living with a relative or friend, the following process should be followed: 

· Completed residency affidavit, with current date and ALL sections complete

· Present category I, II and III items associated with the homeowner identified on the affidavit

· Parent/guardian Driver’s license or state ID

Affidavits can be brought notarized and completed or the affidavits can be notarized at the District Office at no charge for registering families. 


 

Is your lease out of date or are you living on a month-to-month verbal agreement? The following process should be followed:

· The owner/landlord/lessee of the residence must complete the Month-to-Month Rental Agreement Addendum and have it notarized.

· Parent/guardian must bring the notarized agreement addendum along with the original rental agreement at time of registration.

If requiring notary services at the District Office, the parent/guardian along with the owner or lessee must be present for notarization.
