ALL students must have a medical/physical exam form on file according to the specifications noted below.

- Child Health Examination
- Immunization Records
- 4 doses of DTaP
- 3 doses of IPV
- 1 dose of MMR and Varicella
- 4 doses of HIB
- 3 doses of Hepatitis B
- Pneumococcal series or one dose after age 2
Kindergarten
- Child Health Examination
- Immunization Records
- 4 dose of IPV
- 2nd dose of MMR and Varicella
- At least 4 DTaP
- At least 4 Polio
- 3 Hepatitis B
- Diabetic screening
- Dental Exam
- Vision Exam
Grade 2
- Certificate of Child Health Examination Form on File
- Immunization Records
- Must have shots listed for previous grades
- Dental Exam
Grade 6
- Child Health Examination
- Immunization Records
- 1 dose Tdap
- 1 dose Meningococcal vaccine
- Diabetic screening
- Dental exam
Grade7 & 8
- Immunization Records
- All 6th grade requirements should be on file
